We are a licensed addiction treatment provider. When you become a client, your protected health information ("PHI") is also governed by the federal Health Insurance Portability and Accountability Act ("HIPAA") and 42 CFR Part 2. The HIPAA Notice of Privacy Practices at the bottom of this page describes those additional rights and our responsibilities.

Information You Provide
When you contact us through a form, request a callback, verify your insurance, or call our helpline, you may provide us with personal information such as your name, phone number, email address, date of birth, insurance information, and details about your or a loved one's substance use, mental health, or treatment history.
Information Collected Automatically
When you visit the Site we may automatically collect information about your device and browsing activity, including IP address, browser type, operating system, referring URL, pages viewed, and the dates and times of your visits. We collect this information using cookies, log files, and similar technologies.
Information from Third Parties
We may receive information about you from referral sources, healthcare providers, insurance carriers, family members, legal representatives, and analytics or advertising partners.

When you provide your mobile number, you may receive text messages from us related to admissions, scheduling, and follow-up care. Message and data rates may apply. Reply STOP at any time to opt out. SMS consent and phone numbers collected for the purpose of SMS communications are not shared with third parties or affiliates for marketing purposes.
4. How We Share Information
We do not sell your personal information. We may share information in the following limited circumstances:
- Service providers: with vendors who perform services on our behalf (hosting, analytics, communications, EHR, billing) under contractual confidentiality obligations.
- Healthcare operations: with treating clinicians, labs, pharmacies, insurers, and other healthcare partners as permitted under HIPAA and 42 CFR Part 2.
- Legal compliance: when required by law, court order, subpoena, or regulatory request, or to protect our rights, safety, or property, or that of others.
- Business transfers: in connection with a merger, acquisition, financing, or sale of all or part of our business, subject to applicable law.

Your Rights
Get an electronic or paper copy of your medical record
You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. Ask us how to do this. We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee.
